360 Feedback Survey: Selected Respondent List

As part of this development program, we will be administering a short online survey asking the people who work closely with you to provide you with feedback on your leadership skills. You will be asked to rate yourself as well.

We need you to provide us with the names and email addresses of the people who will be taking the survey by filling out an online form. Please gather this information before accessing the form  HERE


This list should include:

• At least one manager—if you have more than one manager, add one to the list

• Four or five peers/colleagues

• Four or five direct reports

 

If you don’t have direct reports, you can select additional people from whatever cohort works for you.

 

Please note: to ensure confidentiality, at least two peers/colleagues must respond to your survey in order for that group to be reflected in your report. The same is true of direct reports, but not of managers.

 

360 Feedback Survey: Draft Text for Notifying Respondents

 

Below is suggested text for a note you could use to alert people to the survey request they will receive.

 

Dear Respondent,

I am participating in a professional development program. As part of this program, I have been asked to provide the names of people I work with who can provide feedback on my capabilities. I have identified you as one of those people and I’m hoping you will be kind enough to respond to an invitation to an online survey that will be emailed to you within the next few days by MOR Associates, an outside consulting firm. This survey is administered by MOR Associates and I will not see individual responses.

Please note that the invitation will be from Brian McDonald and the address will be surveys@mor-assoc.com. Please think about taking steps to ensure that your spam filters see this as a legitimate address.

Thank you in advance for taking the time to help me. I value your candid feedback. If you don’t think you will be able to respond to the survey, it would be helpful for me to know so I can ask somebody else.
